In this position the selected candidate will do a combination of field and remote work.
Part of the time will be spent in member homes completing MDS-HC assessments.
The other part of the time will be spent completing documentation and follow-up calls and processes (utilizing remote home office).
RN's must be located within 30 miles of designated coverage territory.
We are currently hiring for the Metro West area.
This is a FULL-TIME position.

Requirements:
RN with unrestricted nursing license in the state of Massachusetts.
Must live within 30 miles of designated coverage territory.
The ability to drive in designated territory to visit members in their homes and/or other settings, including community centers, hospitals or providers' offices.
Experience working with MS Office including Excel, Outlook, OneNote and SharePoint.
STRONG technical skills is a MUST.
Preferred
Qualifications:
2
years of experience working in a health care role.
Experience in long-term care, home health, hospice, public health or assisted living is always a plus but we will train RN's who are eager to learn.
A background in managing populations with complex medical or behavioral needs is helpful.
Experience with electronic charting.
Experience with arranging community resources.
Prior field based work experience.
Bilingual in Spanish, Russian or Cantonese is preferred but not required Primary
Responsibilities:
Assess, plan and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of care Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services Manage the care plan throughout the continuum of care as a single point of contact Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members Advocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team.
